The Regional Statistical Services Office XI of the Philippine Statistics Authority is pleased to inform you about the Philippine Identification System (PhilSys) registration for Filipino children ages one to four. This aligns with the PSA's organizational goal of providing an inclusive, secure, and sustainable national identification system.

Furthermore, PhilSys registration for children aged one to four requires the presence of a PhilSys-registered parent or guardian. The PhilSys Number (PSN) of the parent or guardian will be linked to the PSN of the child. It is important to note that once the PSN is linked to either parent or guardian, it may no longer be unlinked.

To proceed with PhilSys registration, the accompanying parent or guardian is reminded to bring the original copies of the following documents: their PhilID or ePhilID and identification and/or supporting documents of the child. Any document is acceptable as long as it contains the child’s full name, date and place of birth, and the full name of the parents. In the event that the guardian will accompany the child, an authorization letter and valid identification document of the parent must be presented. However, if  both parents of the child are deceased, the accompanying guardian needs to present a Certificate or Report of Death of both parents of the child in addition to the aforementioned documents.

PSA encourages all parents or guardians to register their minor children aged one to four years old, enabling them to promptly access the benefits of PhilSys registration, such as valid proof of identity for school enrollment and passport applications.
